Apply for a Passport

To apply for your first U.S. passport (for an adult), you will need:

  • A completed passport application Form DS-11.
  • An original U.S. birth certificate, Consular Report of Birth Abroad or Naturalization Certificate.
  • Photo identification (driver’s license, ID card, etc.).
  • All passport applicants must provide one 2″ x 2″ or 5cm x 5cm photo in color on a plain white or off white background. The image must be sharp and correctly exposed. Read more about  photo requirements here.
  • All passport applicants will be required to pay a passport issuance fee.  The fee must be paid in cash or by some types of credit cards (Visa, MasterCard, American Express, and Discover) in either USD or EURO to the Embassy at the time of application submission.

To apply for a U.S. passport for children under 16, you will need:

  • If you need to apply for your child’s passport, you may come to the Consular Section to apply for a passport.  To make an appointment, please click here.
  • A completed passport application Form DS-11.
  • In order to issue a U.S. passport, we must have proof of citizenship. You may submit either U.S. certificate of birth or Consular Report of Birth Abroad.
  • All passport applicants must provide one 2″ x 2″ or 5cm x 5cm photo in color on a plain white or off white background. The image must be sharp and correctly exposed. Read more about photo requirements here.
  • Both parents with their child should come to the Consular Section to apply for their minor child’s passport.  If only one parent can come, then you will need to complete a notarized DS-3053 form. The absent parent should submit a copy of his/her passport with the application. Submit a photocopy of the front and back side of the ID that the absent parent presented to the notary public with Form DS-3053.
  • If the child’s appearance has changed since the last passport was issued, present some other form of photo ID, such as a recently-issued school ID or Lithuanian passport. Please bring a series of family photographs (e.g., for a 6-year old applicant who last passport was issued at age 1, please bring a series of family photos taken at ages 1, 2, 3, 4 and 5).
