To apply for your first U.S. passport (for an adult), you will need:
- A completed passport application Form DS-11.
- An original U.S. birth certificate, Consular Report of Birth Abroad or Naturalization Certificate.
- Photo identification (driver’s license, ID card, etc.).
- All passport applicants must provide one 2″ x 2″ or 5cm x 5cm photo incolor on a plain white or off white background, no glasses. The image must be sharp and correctly exposed. Read more about photo requirements here.
- All passport applicants will be required to pay a passport issuance fee. The fee must be paid in cash or by some types of credit cards (Visa, MasterCard, American Express, and Discover) in either USD or EURO to the Embassy at the time of application submission.
To apply for the first U.S. passport for children under 16, both parents, in person, must confirm their consent for issuing a passport to their child. Both parents with their child should come to the Consular Section to apply for their minor child’s passport. If only one parent can come, then he/she will need to present a notarized DS-3053 form and the ID or passport copy of the absent parent. Submit a photocopy of the front and back side of the ID that the absent parent presented to the notary public with Form DS-3053. Applicants age 16 – 18 must show that at least one parent is aware that you are applying for a passport, therefore at least one parent must appear with you in person when you apply for your passport.
