Lost or Stolen Passport

  • Americans should safeguard passports as they would any other valuable.
  • If your passport is lost or stolen in Lithuania, you may come to the Consular Section to apply for a replacement.  To make an appointment, please click here.
  • In order to issue an American passport, we must have proof of identity and citizenship.
  • All passport applicants must provide 2″ x 2″ or 5cm x 5cm photo in color on a plain white or off white background.

  • To apply for U.S. passport for children under 16, both parents, in person, must confirm their consent for issuing a passport to their child. Both parents with their child should come to the Consular Section to apply for their minor child’s passport.  If only one parent can come, then he/she will need to present a notarized DS-3053 form and the ID or passport copy of the absent parent. Submit a photocopy of the front and back side of the ID that the absent parent presented to the notary public with Form DS-3053Applicants age 16 – 18 must show that at least one parent is aware that you are applying for a passport, therefore at least one parent must appear with you in person when you apply for your passport.
  • All passport applicants will be required to pay a passport issuance fee. The fee may be paid with a bank card or in cash (either USD or EURO) to the Embassy at the time of application submission. You may find the fee calculator here.
